What is High Arched Foot ?
The majority of the time, people who have higher arches have normal functions of the foot. A very small proportion have imbalances of the muscles due to inborn neurological conditions such as HSMN (hereditary sensory motor neuropathy), spinal cord abnormalities, residual clubfoot etc.
What do these patients present with?
Hard callosities in the skin due to the abnormal position of toes, generalized pain in the feet, and regular wearing of the shoes on the outside. Patients complain of frequent ankle inversion injuries.
How does the doctor diagnose?
On examination, the heel is inverted, the Achilles tendon and calf muscles are tight, toes are clawed, and metatarsal heads are dropped down, causing hard skin on the ball of the foot. X-rays of the ankle and feet are done to exclude any arthritis. Sometimes MRI of the spine is done to exclude neurological causes.

Surgery is only needed if the deformity is causing pain and hard callosities make shoe-wearing difficult. If the deformities are flexible, soft tissue operations like heel cord lengthening and tendon transfers, along with bone osteotomies, improve the position of the toes and heel. If the deformities are stiff or painful, a fusion operation is advised to the toes to straighten them or hindfoot joint fusions for deformity correction.
